COVID-19 vaccination scheme (26th Feb. 2021)

Vaccination of all residents in Malaysia has now begun, and will proceed in three phases: Phase 1 (Feb to April) for frontliners and those involved in healthcare, police, fire & rescue, and civil defence; Phase 2 (April to August) for those in high-risk groups, including senior citizens and those with co-morbidities; Phase 3 (May to Feb 2022) for all remaining individuals aged 18 years and above.

We can now register to receive our vaccination via the MySejahtera App on our phones; just update the app and follow the instructions using the ‘COVID-19 Vaccination’ tab on the home page.


If you click on the ‘COVID-19 Vaccination’ tab, there are two pieces of information for you in Malay which you might need some help in translating:

(1) Click on the www.vaksincovid.gov.my and you will find a page labeled LINDUNG DIRI, LINDUNG SEMUA. On this page there is a link to the full pdf in English.


(2) Click on COVID-19 Vaccine Information and you will find a Vaccine Information Sheet and a Vaccine Consent Form. Neither of these have versions in English so I have translated these and give my summaries/comments below. I have only presented the highlights here which you need to know to complete the paperwork. Take this along with you when you attend the designated clinic for your vaccination.

UPDATE… I had my first dose of the AZ vaccine on 7th June 2021, and the consent form looked a little different. However, the main questions are the same (a - d below). Your fill in your name and ID number and phone number first, and all in duplicate. After you have spoken to the doctor about your health status, you then sign the consent part (they will point that out to you) and this is countersigned by the doctor. No need for the ‘Witness Signature’ as shown below. The forms are all in Bahasa Melayu so go prepared!
